In the period from February to June 2022, semistructured interviews were conducted with a national sample of US prescribing providers and registered nurses actively working in the adult emergency department environment, who also use the Epic Systems EHR system. Participants were recruited through diverse channels, encompassing professional listservs, social media platforms, and email invitations to healthcare professionals. Our inductive thematic analysis of interview transcripts involved ongoing participant interviews until saturation of themes was achieved. The themes were established through a process of collaborative agreement.
Interviews were undertaken with twelve prescribing providers and twelve registered nurses. Six themes were found to be related to EHR factors perceived as increasing documentation burden: lacking advanced EHR features, non-optimized EHR design, poorly designed user interfaces, communication difficulties, an increase in manual work, and workflow blockage. Five themes associated with cognitive load were also identified. Two themes prominently featured in the relationship between workflow fragmentation and the EHR documentation burden were the sources behind it and the detrimental effects.
